The worker who acknowledged the policy; the required subject anchor.

The source form template id — which policy-document form produced the record.

Example: 481

acknowledgedTrue or false

Whether the worker's sign-off is present on the record.

Present when the source system records them.

form_titleText

Human-readable template title; the policy document the form belongs to.

The accounting period the acknowledgement falls in, derived from occurred_at.

The revision of the policy that was acknowledged.

Their contents come from the organisation's own forms or systems, so they differ between organisations.

Every remaining form answer, keyed by the tenant's own question text.
