Rework
On a Queensland road job the number is 97 per cent. TMR’s earthworks specification, MRTS04, sets that as the minimum compaction for earth fill in the top 300 mm below subgrade, measured across the lot rather than on one test. The site engineer wants the next layer down today, because the crew and the roller are paid for whether they work or not. The lab result for the layer underneath comes back the next morning. If it fails and the next layer is already on, the engineer now owns two layers: the one that failed, and the one that has to come off to reach it. Rework is not the failed test. It is everything built on top of the failed test before anyone read it.
At a glance
Section titled “At a glance”- What it is
- Work done twice: failed inspections, NCRs, wrong ground assumptions.
- The protection
- The failed result reaches the person who can stop the next stage the day it lands, with the close-out date attached.
- Running today
- Every NCR, test result and plant pre-start lands in one register, tied to the location and the crew - so the pattern shows up before the same defect gets poured twice.
Where it starts
Section titled “Where it starts”Every rework verdict is filed under one of five causes: the stage or control that failed. They come from the same vocabulary the product uses, in the order the app’s disease page shows them.
| Cause | What it covers |
|---|---|
| Design and documentation | Wrong, late, incomplete or superseded design; clashes; unclear specification; an unanswered RFI. |
| Scope and change | A requirement changed after work was committed: client or end-user change, variation, design change, a latent-condition instruction. |
| Planning, procurement and sequencing | Wrong quantity or type bought, repeat orders, over-order, wrong method or sequence, temporary works, long-lead material. |
| Setting-out, ground and external conditions | Wrong level or position, survey error, service strike, ground not as recorded, weather or external damage. |
| Workmanship and quality conformance | Work not to specification: failed test, NCR, ITP or hold-point failure, non-conforming work, punch list, defective material installed. |
What the daily check needs
Section titled “What the daily check needs”The rework check runs once the organisation holds:
- Data on the purchase line register
Registers that feed it
Section titled “Registers that feed it”These registers add evidence to a rework verdict. Each linked name is a published register contract.
- Inspection test result

Next steps
Section titled “Next steps”- The five diseases - the other four, and why they are one problem
- Why the diseases are in this order - why rework comes first
- Obligations - the specifications and clauses rework is read against
- Register catalog - the safety and quality registers
